Yes, you should get it- people with immune system problems are at higher risk from flu. In the US, the H1N1 vaccine does not contain adjuvants such as squalene. It is made exactly the same way as seasonal flu vaccine is made every year. I and my family have already received it.
You would need the flu shot as the Flumist is not given to people with immune problems.
